Wellbe Debuts Guided CarePath for the Perioperative Surgical Home

Provider and patient facing tools specifically designed for step-by-step surgery guidance

Wellbe, Inc. announced today the availability of its Guided CarePath for the Perioperative Surgical Home (PSH).

Wellbe offers the only cloud-based platform that combines vital tools for patient engagement and care team coordination across the perioperative continuum. This patient-specific, coordinated pathway approach ensures higher patient engagement across the episode and results in reduced risks, optimal costs and a better patient experience.

“Traditional care models have typically been a loose collection of ‘silos', where each provider did his or her own thing. This usually led to poor communication between providers and patients, and even poorer communication between the providers themselves. The patient was dragged through the process, rather than being an integral part of it,” said Dr. Scott Springman, Anesthesiologist at University of Wisconsin Hospital and Clinics. “Wellbe’s Guided CarePath is an exciting development in producing a seamless experience for the patient, while also improving medical providers' ability to reduce expensive delays, complications, and length of stay. By channeling care events into a coordinated sequence, both patients and providers are much more satisfied as partners in the process.”

The Guided CarePath is accessed by patients via the web on PCs, tablets or smartphones. The comprehensive program covers onboarding, risk assessments and pre-surgery preparations, inpatient reinforcement, discharge/transitional care planning, and recovery support during the post-acute period.

Patients are enrolled when a treatment decision is made and, like a GPS, the guided program delivers everything the patient and provider needs as turn-by-turn directions at exact moments in the journey of care, including: shared decision-making, self-care instructions, assessment measures, education (including videos), preparation checklists, and feedback surveys.

“Guided episode management helps achieve the PSH goals of having a patient-centered, multidisciplinary, physician-led, team-based system of coordinated care,” said James Dias, CEO of Wellbe, Inc. “Our PSH Guided CarePath is specifically aligned with value-based care redesign efforts to improve the patient experience before, during and after surgery.”

This turnkey system can be tailored for any organization and ready for patient use in less than three months. The program can simultaneously support high volumes of patients across multiple facilities.

Wellbe’s Guided CarePath for the Perioperative Surgical Home will be featured at the OR Excellence conference in New Orleans on October 15-16 in Booth #604.
