Multi-System Hospital Leverages Chenoa's Patient Portal to Enable NCQA Patient Centered Medical Home
Online, May 15, 2014 (Newswire.com) - Chenoa Healthcare Technology Services announced today that a multi-specialty hospital system in Brooklyn, NY selected their web-based Patient Portal solution to enable patients to manage their own healthcare and improve overall quality of care. With the implementation of the portal, the hospital system is recognized as an accredited NCQA Patient Centered Medical Home.
The Patient Portal, an intuitive and fully customizable web-based platform enables patients to have secure access to their medical information, conveniently allowing them to view, track, manage and share medical records such as prescriptions, lab results, referrals, immunizations, procedures, allergies and medical billing. Additionally, the portal enables patients to instantly communicate with their physicians and caregivers, share medical information, schedule appointments, request referrals, receive appointment and medication alerts, view and pay bills, and answer any critical questions - all online.
"Without a patient portal, our hospital was not able to provide patients with access to their medical information," said the Director of IT. "Chenoa's Patient Portal has not only allowed us to become a Patient Centered Medical Home, a long time goal we've been trying to obtain, but it is a vital tool to ensure patients receive preventive and chronic care evidence-based guidelines and improve communications with their physicians and caregivers for shared healthcare decision making."
The Patient Portal is built on HIE and HL7 platforms to offer secure messaging capability and is in compliance with mandated HIPAA regulations. The beauty of the portal is that it's EHR agnostic allowing the hospital system to leverage their existing data from multiple EHR and transactional data management systems requiring no additional EHR staff training. Chenoa's team continues to enhance the portal based on our clients' needs in conjunction with their EHR system. Currently the portal is equipped for ambulatory healthcare settings, but will soon be implementing inpatient care transactional data. Following the model of the Patient Centered Medical Home, the hospital system now meets Meaningful Use Stage 2 requirements, automates the routine tasks of population health management, enhances patient engagement, tracks and coordinates care, and delivers better care and better quality at lower costs.
